What is IRIS Community Living?
Community Living provides 24 hour support in homes for people with physical and intellectual disabilities. IRIS Community Living works alongside people and their families to support them to live a regular life.
Who we serve
Community Living works with people aged 16 years and over, of all cultures, who have a physical or intellectual disability and require ongoing daily support. We also work with families and wider communities to promote social connection and community participation.
Accessing this service
IRIS Community Living is accessed by referral from a Needs Assessment Service Coordination (NASC) agency.
How we work
We have a person centred approach means that we like to understand as much as possible about the person and their goals, and for the person to have as much autonomy and control in how the service is delivered.
Working alongside people, together we determine their needs and goals, what is most important to them and how we can make the greatest difference. Our “can-do” attitude means we are flexible, willing and able to find a way to provide the support people require and to help achieve their goals.
